Meet the Team

Mitchel C. Jacobs, M.D., F.A.C.P. is the Medical Director of Long Island Jewish-Forest Hills. He is responsible for all medical affairs, quality management, safety, and standards of care at this 312 bed institution. Dr. Jacobs has 30 years of experience in medicine and has spent the past 2 decades at North Shore University Hospital-Manhasset. Dr. Jacobs is a board certified intensivist, with specialty in pulmonary medicine. After completing training in critical care at Montefiore Medical Center, he was recruited to be the director of the Cardiothoracic Intensive Care Unit at NSUH-Manhasset. Dr. Jacobs was the first fellowship trained intensivist at NSUH-Manhasset. He established the first 24/7 critical care system, staffed by attending intensivists, in the northeast United States. He subsequently was named the Chief of Critical Care for the Department of Surgery and Director of the Surgical Intensive Care Unit. In this role, he was responsible for directing the teams that rendered care to all critically ill patients that included cardiac surgery, general surgery, trauma, transplant, neurosurgery, as well as all other surgical patients. By establishing quality control metrics, accountability, and efficiency of care in the ICUs, Dr. Jacobs was instrumental in improving the patient outcomes that ultimately led to multiple awards and national recognition for NSUH-Manhasset. He has published in the medical literature and has been a nationally recognized expert in quality management, efficiency, and staffing of Intensive Care Units. He has lectured both nationally and internationally and served on national committees in this regard. Dr. Jacobs is the recipient of the President’s Award from the Society of Critical Care Medicine, American College of Critical Care Medicine. Dr. Jacobs is a graduate of the University of Bologna, School of Medicine. He has previously been an Assistant Professor of Medicine at Weill Cornell Medical College and New York University School of Medicine and currently at the Hofstra Northwell School of Medicine. Dr. Jacobs is dedicated to providing high quality, efficient, compassionate medical care and to improving the patient experience.

Farzin Rahmanou, D.O., F.C.C.P., F.A.C.P. joined Nassau Pulmonary in 2015. He graduated from New York College of Osteopathic Medicine in 2007. After graduation, he completed internship and residency in Internal Medicine. He then continued to a fellowship in Pulmonary and Critical Care medicine at New York Hospital of Queens – an Affiliate of New York Presbyterian Hospital.
After completing training in 2013, he has worked in multiple hospitals within Long Island until he joined Nassau Pulmonary in 2015.
He is certified by the American Board of Internal Medicine in Internal Medicine, Pulmonary Diseases, and Critical Care Medicine. Dr. Rahmanou is also a Fellow of the American College of Chest Physicians and Fellow of American College of Physicians. Dr. Rahmanou is currently the associate chief Critical Care in Glen Cove Hospital. Dr. Rahmanou is fluent in English and Farsi.
After completing training in 2013, he has worked in multiple hospitals within Long Island until he joined Nassau Pulmonary in 2015.

Kristen Porreca is a New York State licensed and ANCC board certified nurse practitioner. She graduated with her Bachelor of Science in Nursing from the State University of New York-Stony Brook. After completing a critical care nursing fellowship, Kristen began working as an RN in the Medical Intensive Care Unit at North Shore University Hospital. She went on to receive her Master of Science in Nursing-Adult Health at SUNY-Stony Brook. She continued to practice at North Shore University Hospital in Manhasset as a nurse practitioner. She started at first for a general medicine service and subsequently in a specialized respiratory unit providing care for those with advanced pulmonary disease and need for ventilator management. While working in the respiratory care unit, Kristen was chosen to serve on a multidisciplinary team in a collaborative initiative with the Institute for Healthcare Improvement to improve care for patients with advanced illness. She has been with the team at Nassau Pulmonary & Critical Care Medicine since August 2015.
